Case Coordination

Most people want to live in their own home as long as possible. Case Coordinators work individually and confidentially with people to determine what services they need to remain living independently. They work one-on-one with a client unless the client wishes to involve spouses and/or other family members. Each client is unique in what services they need and Case Coordinators work with other area agencies to secure those services.

Referrals come from a variety of sources. Sometimes the person calls looking for services. Sometimes children, both local and living long distance, call concerned about their parent. Churches, friends, and other agencies make up the balance of referrals.

Case Coordinators can determine in the initial phone call whether or not the person qualifies for any Federal or State funded programs. If the person’s income exceeds these programs, then Case Coordinators can work with the client on a fee base. Complete assessments are conducted in the client’s home, not over the phone.
